Named GP

From 1st April 2015 GP practices are required to inform patients of a named GP accountable for their care. As all of our patients are registered with Dr Rex Obonna, he will be your named GP.

Please note that this does not prevent you from seeing any of the GPs or locums who may visit the practice from time to time as you currently do.

Summary Care Record

From 1 April 2015 summary information from your medical record will be uploaded to the Summary Care Record (SCR) section of the NHS National Spine.

This is now part of the GP Contract.  If we do not comply we would be in breach of our contract and would potentially not be allowed to provide services to our patients.

The information being uploaded includes details of your current medication and any known allergies and adverse reactions.  This is uploaded on the basis of your implied consent.  Additional information can only be uploaded with your explicit consent.

The SCR will only be viewed by other Health Care Professionals (HCPs) in an emergency or urgent care setting with your explicit consent unless you are unable to give it (for example if you are unconscious).

You are able to opt out at any time, to do this you would need to complete an opt-out form available at the surgery.
